To order official Science Olympiad medals and trophies, please follow the instructions below:
Elementary Medals:
Complete and send form plus payment to Science Olympiad, Two Trans Am Plaza Drive, Suite 415, Oakbrook Terrace, IL 60181 or email form to gjputz@soinc.org or fax to (630) 792-1287 and send payment (purchase order or check) via mail. You may also send your form and pay for your order (or purchase order) with a credit card by following the instructions on the Science Olympiad Store.
Invitational Medals:
Complete and send form plus payment to Science Olympiad, Two Trans Am Plaza Drive, Suite 415, Oakbrook Terrace, IL 60181 or email form to gjputz@soinc.org or fax to (630) 792-1287 and send payment (purchase order or check) via mail. You may also send your form and pay for your order (or purchase order) with a credit card by following the instructions on the Science Olympiad Store.
Regional or State Medals:
The order form for these medals is located in the password-protected section of the website under DIRECTORS. Please use your issued username and password to log in or ask your state director for an access code to download the order form.
